Chief of Detectives Joseph Kenny told reporters that the words “delay,” “deny” and “depose” were scrawled on the ammunition, one word on each of three bullets. A law enforcement official, speaking on condition of anonymity, previously told The Associated Press that the words scrawled on the ammunition were “deny,” “defend” and “depose. The messages mirror the phrase “delay, deny, defend,” which is commonly used by lawyers and critics about insurers that delay payments, deny claims and defend their actions.

Medicaid

Medicaid – text messages show Favre at the center of a scandal dating back to 2020, when a political appointee of the governor and nonprofit head, among others, were arrested for misusing federal welfare funds, some of which benefited Favre personally as well as causes with which he was involved. Between 2016 and 2018, the nonprofit Mississippi Community Education Center doled out $5 million—allegedly at Favre’s request—to the University of Southern Mississippi, where Favre attended and where his daughter also attended and played volleyball, to pay for a new volleyball court, according to Mississippi Today; the nonprofit also paid him $1.1 million to promote a program called Families First. The funds mostly came from a federal welfare grant program administered by the Mississippi Department of Human Services, and both the head of the nonprofit and the former head of the MDHS have been charged with and pleaded not guilty to embezzlement and fraud for their roles in the scheme. Prevacus, a pharmaceutical company that was working on a concussion treatment and which Favre was a top investor in, also allegedly received $2.15 million “in allegedly stolen funds” from the scheme, according to Mississippi Today.

UHG Scandal

The Healthcare Scandal of All Time – Senator Scott – In 1987, Scott started Columbia, which was a hospital company (which later became Columbia/HCA). As the years passed, Scott added hospitals, centers for surgery, and other health locations to the HCA network. In 1997, an investigation was launched after records were seized that were indicative of Medicare and Medicaid fraud. Scott resigned shortly after the investigation started, but he couldn’t escape it altogether. Prior to his resignation, several warrants were issued that granted the federal government permission to search hospitals that were owned by Columbia/HCA. The warrants also included hospitals previously owned by the company. As part of his resignation, Scott received $300 million in stock, $10 million in cash, and $5 million, which was quite the severance pay. It was found that Columbia/HCA billed services such as Medicare, Medicaid, and TRICARE for tests that physicians hadn’t ordered. Scott’s company also trumped diagnoses with fake ones to increase the hospital’s reimbursement. They illegally claimed that advertising expenses were “community education.” The company also billed the government on behalf of non-qualifying patients for healthcare visits at home. According to the Department of Justice, Columbia/HCA was ordered to pay $840 million in fines and penalties for its deceit.

Dog Eats the EOB

UHG – Dental Program – Patient goes to A1 Dental – 3501 Associate Drive | Greensboro, NC – This facility only accepts cash and Medicaid (that’s a clue they may have questionalbe service and billing). Patient pays cash for the service – almost $1,000 in August 2022. The following is a summary of the UHG Dental Claims Process:

  • Dental service provided by A1 Dental in August 2022 – Patient pays out of pocket for OON services.
  • Patient has not received an EOB.
  • Patient had a friend who was employed with UHG years early and assisted said patient, pt was educated about OON benefits but the EOB was requred to proceed.
  • Weeks and weeks passed without UHG uploading the EOB, mailing or emailing (all of which UHG stated would happen).
  • A post was written on social media and immediately as response from UHG was sent – September 2024 a year and a month after the service was performed.
  • Wow DOS August 2023 – Claim has still not been adjudicated for in-network provider at the time of service and no EOB has been sent it is now December 2024 – Claim was worked on by a dedicated Dental claim analyst who is unable to get the dental codes for the service provided by A1 Dental in Greensboro, NC even AFTER the Dental Commissioner of NC was requested for assistance.
  • Additionally it was disclosed that the provider of service had just been authorized to render services after voluntarily surrendering his licenses for violating various dental practices in North Carolina.

Not Fired Not Laid Off Just Gone

UHG Employee in Trumbull has a hysterectomy and while on Medical leave the supervisor who was a co-worker one day, supervisor the next day – right after requesting the answers to the computer program to review claim issues for individual providers, group practices and hospitals. While on medical leave, [supervisor] calls employee and requests computer used to work from home that’s it. When said employee contacts supervisor no response or reply is provided to employee.True Story – employee prefers to remain anonymous – UHG can contact this writer for additional information.
